In-home patient care option for Burlington and surrounding communities
The Community Remote Care Management program connects patients with a multi-skilled care team using an app on a tablet or smartphone. The app makes it simple to communicate and securely share health information.
The Burlington-area program is for:
- Patients with chronic obstructive pulmonary disease (COPD) and/or congestive heart failure (CHF) and/or vulnerable seniors.
Using an app called Aetonix aTouchAway, patients with COPD and/or CHF can:
- Connect to their care team via messaging and virtual visits
- Respond to reminders
- Learn more about their condition
Patients being monitored because of their chronic conditions borrow a free tablet and medical monitoring equipment so they can:
- Measure and send current vital signs (such as pulse rate or temperature) and symptoms to the care team
- Avoid unnecessary trips to the clinic or hospital
The Community Remote Care Management team is a collaboration between Burlington Family Health Team professionals (including a Nurse Practitioner and Registered Practical Nurse), Halton Region (Community Paramedic), and Thrive Group (Community Connector). Additional Burlington Family Health Team professionals, such as Respiratory Therapist, Physiotherapist, etc. are called in when needed to build a coordinated care team for the patient and family. The patient’s primary physician is informed and involved regularly.
The Community Remote Care Management program provides comprehensive, continuing patient care from the safety of the home environment. It was designed by several partners of the Burlington Ontario Health Team and provides an excellent experience for patients and healthcare providers, peace of mind for family members, and better health outcomes.
