What is the Community Wellness Hub?

It’s a publicly funded program for older adults at designated affordable seniors housing buildings in Burlington and Oakville. The Hub provides residents with health and social care by professional health and wellness community partners.

The Vision

To keep seniors who are at risk of hospitalization or institutional care living healthily and happily at home for as long as possible.​

The Mission

To offer care and support through trusting relationships and an integrated approach that proactively identifies and responds to physical, mental and social “flags” before individuals get into crisis where they require acute care resources.​

Services include:

  • One-to-one support to connect to local health and wellness services.
  • A community connector who helps coordinate your care.
  • Social events, exercise classes, and recreational activities to stay active and make new friends.
  • Workshops about healthy living, mental health, managing medications, etc.
  • Tablet device loans and Internet so you can keep in touch with friends and loved ones.

The Hub supports you and your circle of care. What does that mean?

The circle of care refers to the Hub team members who provide you with services, and others involved in your health and wellness. With your approval, information about you and your health needs are shared with your circle of care. This helps to avoid having to tell your story every time you meet a new service provider. It also helps your circle of care to work together to support you.
