The Burlington Community Physicians Council has transitioned to the Burlington Primary Care Network, and we are revamping our website to reflect these changes and offer our physicians and providers a space to access resources, connect, and stay up to date.

The BCPC was made possible by some truly dedicated physicians in our community and your participation in this community. We are committed to honoring that legacy as we move forward together under the PCN.

A Message from the Burlington Community Physicians Council and the Burlington Primary Care Network Leadership Council

Dear Colleagues,

We are in the midst of significant changes in healthcare delivery. Having navigated a challenging few years with the pandemic, we have laid some foundational steps in our community through the Burlington Community Physicians Council. We have been reflecting on our BCPC journey thus far and contemplating how we move forward to become a united primary care voice in Burlington that can lead the changes our system needs.

This month we are pleased to announce a new chapter for primary care in Burlington: the Burlington Community Physicians Council will transition to the Burlington Primary Care Network (PCN). This is our ongoing commitment to meeting community needs by supporting Burlington’s primary care providers through improved service navigation that looks to reduce the burden on our physicians and improve our patient care.

As the Burlington PCN, we look to honour the work of the BCPC and build a network where we can support one another and mentor new primary care providers. The PCN will serve as a central hub, offering essential tools, education, and opportunities for collaboration and advocacy. Led by the Primary Care Network Leadership Council, the PCN will drive initiatives, advocate for your needs, and align with our community’s clinical priorities.

We are looking forward to embarking on these next steps together!
