Burlington's Primary Care Network (PCN) is a collaboration designed to bring together our primary care physicians and other providers in a connected and coordinated way.

As a PCN — including family physicians, nurse practitioners, specialists, allied health professionals and other community service providers— our mission is to improve access to clinical resources, digital tools, education, team-based care that complements your practice and networking opportunities, ensuring our primary care community has the resources and support it needs to deliver better patient care.
