The Ontario Ministry of Health introduced Ontario Health Teams (OHTs) as a new way to delivering health care for patients. Ontario Health Teams will make it easier for patients to navigate the health care system with primary care, mental health care, long-term care, hospital care, social service providers all working together.

Ontario Health Teams will be responsible for delivering care for their patients, understanding their health care history, directly connecting them to the different types of care they need, and providing 24/7 help in navigating the health care system.
