Seamless Care Optimizing the Patient Experience (SCOPE) provides Primary Care Physicians and Nurse Practitioners with a single, centralized point of access to high quality hospital and community care for their complex and/or urgent patients.
The model has been successfully implemented across multiple Ontario Health Teams (OHTs) and is recognized for reducing emergency department visits, improving referral efficiency and enhancing patient and provider satisfaction.
