System Navigators Community of Practice
The System Navigators Community of Practice is dedicated to advancing the role of navigators within health and social services across Halton Region and Hamilton. By recognizing navigation as a distinct yet collaborative discipline, the community of practice aims to improve access to care and streamline support for people seeking it.
This initiative brings together professionals whose work involves guiding clients through complex systems of care. Through regular engagement, members foster continuous learning, share best practices and build stronger inter-agency relationships. Meetings offer opportunities for case presentations, guest speakers and agency spotlights, with post-session materials distributed as a newsletter to keep members informed and connected.
Participation is open to individuals working in health and social service organizations whose responsibilities include significant navigation functions. Rather than a formal leadership structure, the community operates through shared contributions, with members stepping in to support coordination and planning as needed.
To facilitate collaboration, Burlington Ontario Health Team maintains an up-to-date directory of member navigators and their affiliated organizations. This resource helps strengthen connections and ensures that professionals can easily identify and engage with peers across the region.
Community navigation is a strategic approach to connecting individuals with essential support systems across public services, social services, healthcare and the broader community. It encompasses outreach, education, and technical assistance to promote awareness and engagement with local programs.
